Your Home Team

More than in-home nursing or in-home rehab, Avanti Home Health Services takes a team approach to your skilled medical needs. Home Health Services

The reasons patients use Avanti Home Health Services are as varied as the reasons people use a nursing home or rehab facility. Here are a few examples of the care we can provide.

Not Just Medical Care - Life Care

The Avanti Home Health philosophy is that patients in home health need more than focused attention on the primary diagnosis. More than caring for your disease, we provide medical care for your quality of life. For instance, if an Avanti nurse is checking on an elderly patient at home with pneumonia, we also assess for the strength and ability loss that can occur with bed rest. When there has been strength loss, we send a physical therapist to develop the program that will get our patient back to full ability as soon as possible. We may send an aide to help around the house while our patient is regaining strength. If strength recovery is going to take a long time, we can send an occupational therapist to teach new ways of doing things independently at home - to minimize the burden on family caregivers.

Often, a person's need for in-home support extends past the time that health insurance will pay. Long-term care (and especially long-term support) exceeds the scope of normal health insurance, but that doesn't mean Avanti Home Health stops helping you there. Our private duty home care can transition your care from interim medical assistance to long-term support and explore other payment options such as Medicaid waiver services, veterans' benefits, and private pay plans.

When we look at a patient as a whole person, no one diagnosis stands in isolation. Every medical situation involves other diagnoses, other health considerations, the home environment, family, finances, and more. Our multidisciplinary team stands prepared to see and care for patients as whole people.

If you or someone you care for could use the services of home health aides, nurses, or therapists, please contact us now to discuss your needs.
